If you wish to participate in the Canadian benefits program, enrollment forms must be completed and submitted within 31 days of your date of hire. Your coverage is effective on the date of your application. If you enroll after 31 days, evidence of insurability will be required prior to participation.
In order to process your enrollment form, Windstream must have an original copy of your signed enrollment form. The form must be mailed and postmarked within the 31-day enrollment window and sent to:
