Biometric Screening

Windstream supports the well-being of our employees and family members with a full menu of resources designed to help you maintain or improve your health. One of these resources is a biometric screening.

As a US-based employee, you are eligible for a free, confidential biometric screening as part of the Windstream well-being program.

In prior years, a biometric screening was required for employees and their spouses enrolled in a Windstream medical plan to avoid a medical premium increase. The screening is not required for 2023. We still encourage employees and their spouses to visit a Quest lab or their physician to obtain a screening to ensure their health is on track.

A biometric screening is an abbreviated health examination that provides key health indicators, helps determine your risk level for certain medical conditions, and helps monitor changes against your personal health baseline over time. Although biometric testing does not definitively diagnose conditions or diseases, a screening creates awareness of potential health risks and gives you information to improve your health.

Employees have access to a Windstream paid biometric screening through Quest labs regardless of medical plan enrollment. Employees and spouses enrolled in a Windstream medical plan can also visit their primary care physician for a biometric screening paid by the Windstream medical plan.

Please take a few minutes today to invest in your health by scheduling your screening. You have two options to complete your free biometric screening:

Option 1: Visit a Quest Lab

• NEW users – Select ‘Register Now’ if you are a new user and enter Registration Key: Windstream2023
  
  Enter your unique ID:
  – For employees it is your employee ID (without the “E”) including leading zeros.
  – For spouses it the employee’s ID with an “S” at the end of the number.
  
  RETURNING users – Use last year’s log in information and reset it if necessary (please contact Quest at 855.623.9355 for any log in issues).
• Choose “At A Patient Service Center” to schedule a visit to a Quest lab near you.

Option 2: Complete your Screening through your Doctor (available for employees and spouses enrolled in a Windstream medical plan)

  Enter your unique ID:
  – For employees it is your employee ID (without the “E”) including leading zeros.
  – For spouses it the employee’s ID with an “S” at the end of the number.
  
  RETURNING users – Use last year’s log in information and reset it if necessary (please contact Quest at 855.623.9355 for any log in issues).
• Click the “Physician Form” button and proceed through the site to download and print the form; then take it to your physician to fill out.
