At Windstream, we realize that simple, everyday choices have the power to impact our well-being—from our emotional and physical health to our job performance, health-related costs and much more. That is why we encourage you to participate in the new Be Balanced Program and make your everyday choices count toward your well-being.

         
           

What is Be Balanced Program?

The Be Balanced Program is Windstream’s global well-being program. It includes a full menu of activities to help you maintain or improve your health in areas that may be identified through a biometric screening and a Well-Being 5 Assessment (WB5) and focus on your overall well-being. Employees AND spouses who enroll in a Windstream Medical Plan who complete BOTH the biometric screening and WB5 between 2/1/2018 and 5/31/2018will maintain a lower medical premium.

There is a $500 annual surcharge for not completing those activities. The surcharge will be applied in July. New hires are excluded from the surcharge in their first calendar year*.

How to Participate in the Be Balanced Program: 

Complete your 2018 Biometric Screening by May 31, 2018 

Unlock all of the features of Be Balanced by completing the WB5 assessment first. It’s okay if you don’t know the answers. Biometric data will be corrected when your screening loads.

A biometric screening is a quick and easy check-up that helps evaluate your health and pinpoint health issues before they become detrimental. A health care professional collects your height and weight measurements, waist measurement and blood pressure. Then he or she administers a blood test measuring cholesterol, triglycerides and glucose levels. Click here for more.

Complete the WB5 Assessment by May 31, 2018 

The WB5 Assessment is a questionnaire that assesses life, environmental and financial factors that are critical to your health, well-being and your ability to improve both. The assessment has been designed by health experts and consists of questions to get a better sense of your overall health and well-being, including:

• Purpose • Social • Financial • Community • Physical

The QuitNet® Program 

The QuitNet® program is here, supporting your efforts to quit tobacco for good at no cost to you. If you use tobacco, quitting can improve your health and give you back precious time and money. Take back what’s yours with help from the QuitNet program. It’s the world’s largest tobacco cessation program, helping people quit cigarettes and other forms of tobacco. Connect with a community of quitters who know just what you’re going through and kick the habit for good. Click here for more.

A $50 monthly surcharge will be applied to employees and/or spouses who attested to using tobacco products during enrollment and did not commit to completing the tobacco cessation program in 2018. The surcharge can be stopped prospectively during the year by completing four smoking cessation coaching calls through the QuitNet Program or an alternative coaching program between 2/1/18 and 11/30/18*. To remove the surcharge, you will need to contact Mercer Marketplace at 866.553.9409.

Well-Being Connect 

Well-Being Connect™ (WBC) is a dynamic online portal designed to inspire improved health and well-being. The online portal will help you live more fully by getting to know you better, helping you set goals and connecting you to support to achieve your goals. Health Coaching 

Based on your responses to the questions you completed on the WB5 and the values presented from your biometric screening you may be selected by Healthways/Sharecare credentialed health professionals to participate in telephonic health coaching. Telephonic health coaching is a voluntary program, offering personal support from a health coach who can help individuals with moderate to high health risk factors overcome the challenges that stand in the way of their good health. Whether you’re setting goals to lose weight, managing an ongoing lifestyle issue, like quitting smoking or just want to learn more about the wellness program, a coach allows you to focus specifically on your questions.  Click here for more.

*Your personal health information is private and is never shared with Windstream. Healthways/Sharecare only provides information (such as program activity points) involving your participation in order to administer the program. The WB5 assessment and Biometric Screening must be completed by employees and spouses on a medical plan as of 1/1/18 before 5/31/18 to avoid a medical premium surcharge Those who join a plan after 1/1/18 are not subject to the non-completion surcharge in 2018. A $50 monthly surcharge will be applied to employees and/or spouses who attested to using tobacco products during enrollment and did not commit to completing the tobacco cessation program in 2018. All employees are encouraged to participate in the Be Balanced program; however certain labor organizations are not subject to these requirements.
